Please read this Registration form carefully. The Registration Form contains important information regarding the terms and conditions of the Student’s enrollment with NSA North Shore Academy Ltd. (at times referred to herein as “North Shore Academy”)  and participation in the 2026-2027 school year. By signing and submitting this registration form, you are entering into an agreement with North Shore Academy. Among other things, you will be responsible for all costs and fees for the 2026-2027 school year described herein, payable in accordance with the terms of conditions of the Registration Form.

Impromptu Walking Field Trip

As education is not confined to the walls within a classroom, curriculum can be enhanced with field trips outside the classroom and beyond the school campus.   Walking field trips are often spontaneous, and may include but are not limited to the use of a community field, environmental clean-up, change of learning environment, community walk, or jog for exercise.  I understand the impromptu nature and inherent risks of a walking field trip.

Permission to Leave

North Shore Academy offers Student's the opportunity to leave school premises during breaks and/or other school events at their own risk during the 2026-2027 school year with consent.

Permission to Leave Authorization

I GRANT permission for my Student to leave North Shore Academy at their own risk during the 2026-2027 school year. I further waive any and all claims I may have against NSA North Shore Academy Ltd. from all liability and agree not to pursue any legal remedies against NSA North Shore Academy Ltd., its officers, employees, agents, volunteers, and representatives for any personal injury, illness, death, property damage, or loss sustained as a result of my Student's attendance at North Shore Academy.

I do NOT grant permission for my Student to leave the North Shore Academy school premises for breaks and/or other school events at their own risk during the 2026-2027 school year.

Academic Release

Academic success is an integral component of North Shore Academy. In the event of an academic decline or negative change in work habits, I understand that my Student may be required to attend additional academic/behavioural support sessions that may conflict with external interests/athletics. North Shore Academy staff would like to communicate with external providers should a situation arise, advising them of the action plan and timeline needed to positively improve academic and behavioural achievement.

Academic Release Authorization

YES, I authorize North Shore Academy staff to communicate directly with my Student's external providers should a situation arise.

Media Release

I hereby grant or deny (as marked by my selection below) to and for NSA North Shore Academy Ltd. to use images of my 
student, as the owner of NSA North Shore Academy Ltd. determines, such use to include the taking, recording, photographing, display, distribution, publication, transmission, or other uses of photographs, video recordings, digital records and other images taken or made during the course of my student’s activities at or with NSA North Shore Academy Ltd. Such uses may include, but are not limited to, advertising and promotional materials, brochures, newsletters, and electronic media including the NSA North Shore Academy Ltd. website.

Media Release

I, GRANT unrestricted permission for my student's image to be used in print, video, and digital media. I agree that these images may be used by NSA North Shore Academy Ltd. for various purposes and that these images may be used without further notification.

I, DENY permission to use my student's image at all. (Please note some events are video recorded; therefore, your Student will not be asked to participate in these events.)

Liability Waiver

I hereby waive all present and future claims for any loss, damage or personal injury/illness sustained by my Student, including but not limited to an accident,  injury, or illness sustained by my Student (the Student) during classes, or events and I further release, remise and completely discharge the owners, teaching staff, guest faculty, substitute instructors, promoters, directors, principals, and employees of NSA North Shore Academy Ltd. or any Academy partner, or event attended by NSA North Shore Academy Ltd. from any and all liability whatsoever for any injury, illness or loss caused as a direct result of any act or omission by any of them.

Upon completion of this Registration Form, an invoice as per the payment plan selected will be sent to the email address(es) indicated above.
North Shore Academy registration is not complete until all required documentation has been provided and Academy and Registration fees have been paid in full.

Below please find a non-exhaustive list that includes, but may not be limited to, some of the additional fees ("Additional Fees") the Parent/Guardian(s) will be responsible for over the 2026-2007 school year that are in addition to the Academy and registration fees:
- Private or group tutoring,
- Counselling,
- Post-secondary advisor meetings,
- Apparel,
- Concession.

*Additional fees will be billed separately of academy fees, are the Parent/Guardian(s) responsibility, and are due within 14 days of invoice.

Late Payment Policy

A $50.00 late payment fee will be charged for any late, returned or declined payments.

I understand that failure of the Parent/Guardian to pay any of the Academy Fees or Additional Fees as they become due and owing, may impact access to facilities, resources, and activities.  

North Shore Academy reserves the right to suspend any Student whose account is outstanding for more than 14 days.

Cancellation Policy

In the event the Student withdraws, is suspended, or is removed from the North Shore Academy, for any reason, including for failure of the Parent/Guardian to pay the Academy Fees and/or Additional Fees due and owing, between the date of registration and the end of the 2026-2027 school year (i.e., June 18, 2027), all Academy Fees and all Additional Fees regardless of whether or not they have been invoiced are due in full North Shore Hockey Academy. No refunds or credits will be given for any fees, or payments paid between the registration date and the date of withdrawal or removal.

*In the event of government or unexpected closures, no refunds or credits will be issued. Programming will continue with the use of alternative platforms.
